Routine Physicals
A routine physical is an annual health check visit that lasts 30-60 minutes. During this appointment, patients can expect their physician to focus on age-appropriate preventative measures and perform the following:
Review/update personal health history and family history
Review/update current medications and supplements and address refill needs
Address new health issues or acute illnesses
Assess immunization history and administer any appropriate vaccinations
General physical exam
Evaluation for appropriate heath screening measures based on personal and family history
Schedule appropriate follow-up or referrals based on new health concerns and/or chronic medical conditions
Women’s Health
A well-woman exam is an annual visit, which usually encompasses all of the components of an annual physical, but focuses on issues unique to women. This focus includes preventative screening of gynecological and breast diseases. In addition, a well-woman exam may include the following:
Full gynecological history including a sexual history and appropriate screening for sexually transmitted infections
Pre-pregnancy counseling and preparedness, if applicable
Breast exam
PAP smear
Pelvic exam
Age-appropriate screening measures (mammogram and bone density scan)
Diagnosis and Management of Acute and Chronic Medical Conditions
Generally considered a “sick” visit, this appointment may include:
Evaluating any new and undefined concerns or symptoms and/or follow-up on any previously diagnosed conditions
Obtaining pertinent lab work or diagnostic imaging
Adding or adjusting medication dosage
Referrals to a specialist for further evaluation (e.g., diabetes, hypertension, hyper/hypothyroidism, sinus infections, urinary tract infections, back pain, shortness of breath, abdominal pain and injuries)
Diagnosis and Management of Acute and Chronic Medical Conditions
Management of Acute and Chronic Medical Conditions
Medication reconciliation
Follow up after treatment
Referral to specialty
Weight Management
For weight management, this visit is a general health assessment, including certain body measurements (i.e., BMI, weight circumference) and relevant blood tests (i.e., cholesterol panel, fasting glucose). Thereafter, individualized weight loss goals are created as well as additional strategies for weight loss, such as weight loss programs, medical therapy, dietician consultations and referrals for bariatric procedures. Appropriate clinical follow-up will be scheduled to encourage adherence to weight loss goals for success.
Health Promotion and Preventative Medicine
This visit is generally for healthy patients to create a relationship with their primary care physician, which will subsequently allow for better coordination of care. Additionally, these visits focus on maintaining good health and ensuring appropriate and applicable health screenings are performed and up-to-date. Examples of health maintenance measures include: blood pressure screenings, cholesterol screenings, pap smears, mammograms, colon and prostate cancer screenings, and immunizations.
