Southwestern Initiative for Sports Health, Richardson TX

The Southwestern Initiative for Sports Health (SWISH) was founded in 2009 with a mission to continually advance the field of Sports Medicine through education, collaboration, patient care and research. Soon after its founding, SWISH members established the FRESH (Foundation For Research in Sports Health) Fund at UT Southwestern Medical Center. Proceeds from this fund are used to support research efforts geared towards advancing the knowledge of musculoskeletal and sports related injuries. These research efforts include both basic science and clinical studies aimed at “bringing the bench to the bedside” and providing our patients with state-of-the-art strategies for both the prevention and treatment of various injuries.

By involving medical students, resident doctors in training, and other healthcare professions, these research efforts also instill the passion for knowledge and medical advancement in our physicians of tomorrow. Through a generous donation to the FRESH fund by one our patients, a database has recently been created that will allow all SWISH members to track their patients progress longitudinally, and thus track and improve patient outcomes for years to come. Donations such as this are essential to the SWISH research efforts and the advancement of medicine in general.

Having performed over 3500 Metal-on-Metal Hip Resurfacings over the course of 20 years, he is the most experienced surgeon in this field.

Gone are the days when young, active patients crippled from hip arthritis were told to shut up and put up with it. Thanks to the pioneering efforts of Mr Derek McMinn, there is now an enduring solution to victims of this generation – The Birmingham Hip Resurfacing.

Mr McMinn graduated from St Thomas’ Hospital in London, gaining the Cheselden Medal & 1st Prize in Surgery and moved to Birmingham and on to some of the top joint replacement centres around the world to train under renowned surgeons such as Bucholz, Engelbrecht, Insall, Wagner, Ranawat and Harris, learning the finer points of primary and revision surgery.

Working as an Orthopaedic Consultant and as the Director of the Royal Orthopaedic Hospital in Birmingham, he developed a passion for complex hip and knee reconstructions. And, often confronted by the poor results of conventional hip replacement in young patients, he took up the challenge of developing an alternative treatment.

Mr Derek McMinn MD FRCSA few of the many contributions of Mr McMinn to arthroplasty include the introduction of the McMinn Resurfacing in 1991, the Birmingham Hip Resurfacing (BHR) in 1997, the Birmingham Mid-Head Resection (BMHR) device in 2003 and the Birmingham Knee Replacement (BKR) in 2007. In addition he has developed other high performance hip and knee implants and instruments to provide a comprehensive range of solutions for the whole spectrum of joint disorders. Alongside the development of these devices, he has perfected the surgical techniques for their safe implantation. He has personally performed over 3500 hip resurfacings and over 6000 total hip replacements, restoring the quality of life in many young and active patients including several high profile athletes. Surgeons come from around the world to observe and train with Mr McMinn in Birmingham.

There is constant demand for Mr McMinn’s expertise at Orthopaedic Conferences around the world to address surgeons and to present his ongoing research work. In addition to receiving honours of distinction from several quarters, the University of Birmingham has conferred on him the degree of Doctor of Medicine Honoris Causa in recognition of his sterling contributions to the medical profession.

Mr McMinn’s passion for meticulous surgery and precise implantation are reflected in his unparalleled success rate. Visiting surgeons have expressed time and again that it is a treat to watch him operate.

Today there is study after scientific study proving the effectiveness of Mr McMinn’s techniques… and story after individual success story coming from his patients.

Rheumatology Care Consultants

Rheumatoid arthritis is a chronic inflammatory disease in which the lining of the joints become inflamed, causing pain, swelling, and stiffness. It is as an ‘autoimmune disease’ because it occurs when our immune system, which normally fights against infection, starts destroying healthy joints. Severe rheumatoid arthritis can be very painful and even deform or change a joint. It also affects a person’s ability to perform routine activities.

The exact cause for rheumatoid arthritis is unknown and there is no definitive cure. However, with better understanding of the disease helps in better management of disease by relieve the pain and other symptoms, and retarding the progression of the disease. Early diagnosis and treatment is important to minimize the damage to joints.

Management of Rheumatoid Arthritis

Several treatment modalities are available for the management of rheumatoid arthritis and they include medications, physiotherapy, occupational therapy, and surgery.

Medications: There are different types of medications which include disease-modifying anti-rheumatic drugs (DMARDs)-methotrexate, infusion and injection biologic agents; pain medications such as non-steroidal anti-inflammatory drugs (NSAIDs)-aspirin, ibuprofen, and COX-2 inhibitors. The DMARDs reduce the body’s immune response against the joints whereas the NSAIDs reduce swelling and pain.

Physiotherapy: Physical therapy exercises should be done regularly to increase the strength of muscles and flexibility of joints. Transcutaneous Electrical Nerve Stimulation (TENS) is a type of electrotherapy that is given to provide pain relief.

Occupational Therapy: Occupational therapy helps to perform daily activities at home and at work independently through the use of equipments. It also helps you to adapt to your condition using the relaxation and stress-management techniques.

Surgery: Surgical treatment is considered if you have severe rheumatoid arthritis and your symptoms do not get better with the conservative treatments. The benefits of surgery are pain relief and improvement in joint function.

Dr. Jeremy Simon Stanton, Shoulder Elbow Hand Surgeon, Essex

I work as an orthopaedic upper limb surgeon with an elective practice involving the shoulder, elbow, wrist and hand. I perform many arthroscopic shoulder procedures (often in the younger population after sporting injuries) including shoulder stabilisations, SLAP and rotator cuff repairs, sub-acromial decompressions and capsular releases for frozen shoulders and also shoulder arthroplasty procedures for both osteoarthritis and rheumatoid arthritis.

I also carry out both arthroscopic and open operations on the elbow, including elbow joint replacements for both arthritis and trauma in the elderly.

I treat many patients with radio-carpal and inter-carpal arthritis in the wrist and also the not uncommon problem  of patients with an adverse outcome after distal radial fracture.

I have an orthopaedic hand practice, treating Dupuytren's both open and percutaneously, basal thumb arthritis both by trapezial excision and joint replacement, interphalangeal and metacarpo-phalangeal arthritis with joint replacements, and the smaller conditions such as carpal tunnel decompression, trigger finger and deQuervain's release.

I work closely with very experienced anaesthetists such that the majority of my upper limb surgical procedures can be carried out under regional anaesthetic, avoiding the side-effects of a general anaesthetic, a quicker post-operative recovery and hence faster discharge. The majority of my cases can go home the same day following surgery, having been admitted the morning of surgery avoiding the need to spend any nights in hospital.

I also have a general trauma practice and take part in a 1 in 8 on call rota with a weekly trauma list. I regularly carry out fixation of complex proximal and distal humeral fractures using the most up to date methods, and treat many complex distal radial fractures both in the acute and chronic (malunited) stage.

Although an upper limb surgeon, I am also experienced in the treatment of routine lower limb trauma, including hip fractures, femoral and tibial shaft fractures (by plate fixation, intramedullary nailing and external fixation), and ankle fractures.
